Thank you for attending the QPRT Suicide Risk Management Inventory (c) Training Program! This program is taught in approximately 8 hours in a classroom or seminar setting, either in a single day, or over several sessions. Attendance at this training program is the first of a four-step program to help you earn a basic competency credential in suicide risk assessment and risk management.

These four steps are:

  1. Attendance at the training program
  2. Successful passing of the post-course quiz
  3. Successful completion of a supervised role plays in the application and documentation of a QPRT Suicide Risk Management Inventory (c) interview compared to a standard recommended by the QPR Institute
  4. Successful completion and documentation of three QPRT Suicide Risk Management Inventories with actual suicidal consumers as evidenced in a medical records review that meets a standard recommended by the QPR Institute.

Steps 3 and 4 are typically conducted by your employing agency.

When you pass this quiz, you will receive a Certificate of Course Completion from the QPR Institute which states:

(Your name) has completed 8 hours of suicide risk assessment training in the use of the QPRT Suicide Risk Management Inventory © and successfully passed a competency knowledge quiz on this subject matter.

To receive your Certificate of Course Completion you must:

  1. Sign up for the quiz/certificate and make payment
  2. Once payment has been received, the QPRT Quiz Entry Page will open.
  3. Complete the required data fields and take the quiz. Feedback is instantaneous, and you may retake the quiz as many times as you like. We do not provide answers to failed items.
  4. Upon successful passing of the quiz, a Certificate of Course Completion with your name and the name of your Certified Instructor will be mailed to the address you provided.

Your agency may request or require that you print the test score page and give it to your supervisor or other persons as directed. For test security reasons, the quiz cannot be copied and/or downloaded. Quiz items are changed from time to time, and randomly reordered on an intermittent schedule.

* We recommend that you read the QPRT User's Manual you received in your training program before taking this quiz. If you received 7 or fewer hours of continuing education credit for your classroom attendance in the QPRT course, this Certificate of Course Completion reflects a full 8 hours of CEUs, as you will have read the User's Manual in addition to have attended the course.
